26 September 2025, Dosing and product handling
Default final order, $3,500 civil penalty. The order finds that on 21 June 2024 a client in an individual session received 70 to 85 mg of psilocybin analyte, above the state maximum of 50 mg per session (OAR 333-333-5240(1)), and that licensee representatives changed secondary doses for 8 group clients mid-session, hand-ground whole fungi into tea and did not use consent forms or sealed packaging (OAR 333-333-5170).
